This article explains what happens when you resume your Levity subscription after a gap in treatment. Use this article to understand when you may need to restart at the initial dose, when to provide proof of treatment from another provider, and what information must be visible on your prescription label.
Restarting after more than 2 weeks since your last injection
If it has been more than 2 weeks since your last injection, you will need to restart treatment at the initial dose.
This restart process helps the Levity clinical team support safe and appropriate treatment after a gap.
If you continued treatment with another provider
If you continued receiving this medication from another provider during your break from Levity, you must send proof of treatment to Levity Customer Care.
To contact Levity Customer Care:
Go to any Levity page.
Click the lavender box with the black chat bubble in the bottom-right corner of the page.
Send a message explaining that you continued treatment with another provider and need to submit proof of treatment.
Proof of treatment requirements
To provide proof of treatment, submit a photo of your current prescription label.
Make sure the prescription label photo clearly shows:
Your name
Prescription date
Medication name
Dose and strength
Date of your last injection
Important dose restart policy
If Levity does not receive confirmation that you continued treatment elsewhere, the Levity clinical team will restart you at the initial dose.
