Completing My Required Monthly Check-In Consultation

This article explains why Levity requires a monthly check-in consultation, when you will receive your monthly check-in reminder, how to access the monthly check-in, what information you need to provide, and what happens after you submit your monthly check-in. Use this article if you need help completing your required monthly check-in, reporting side effects, requesting a dosage adjustment, or avoiding refill delays.

Why is the monthly check-in important?

Your monthly check-in helps Levity clinicians monitor your treatment progress and ensure your medication remains safe and appropriate for you.

Monthly check-ins are important because they help the clinical team:

  • Monitor your weight loss progress

  • Review side effects or symptoms

  • Assess your response to treatment

  • Identify safety concerns early

  • Determine whether your dose should stay the same, increase, or decrease

Providing accurate and complete information helps support safer prescribing decisions and more personalized treatment adjustments.

When will I receive my monthly check-in reminder?

Levity typically sends a monthly check-in reminder approximately 1 week before your next scheduled order date.

You may receive an email with a subject line similar to:

  • “Your Monthly Check-In Is Due”

  • “Levity Follow-Up Consultation Reminder”

These reminders help ensure you have enough time to complete your check-in before your next refill is processed.

How do I access the monthly check-in?

To complete your monthly check-in:

  1. Log in to your Levity patient dashboard

  2. Navigate to the “Monthly Check-In” section

  3. Select the “Start Now” button

  4. Complete the questionnaire and submit your responses

The check-in questionnaire is completed entirely online through your secure Levity account.

What information will I need to provide during my monthly check-in?

The monthly check-in questionnaire may ask about:

  • Your current medication dose

  • Missed doses or treatment interruptions

  • Side effects or symptoms

  • Weight loss progress

  • Appetite and energy levels

  • New medications or medical conditions

  • Recent lab results if applicable

Reporting side effects and symptoms

You should report any side effects you are experiencing, including symptoms such as:

  • Nausea

  • Vomiting

  • Dizziness

  • Fatigue

  • Constipation

  • Stomach discomfort

You should also report any more serious or concerning symptoms promptly, including symptoms such as:

  • Chest pain

  • Severe abdominal pain

  • Difficulty breathing

  • Persistent vomiting

  • Signs of dehydration

Requesting a dosage adjustment

During your monthly check-in, you may request a dosage increase or decrease if appropriate.

If requesting a dose adjustment, provide as much detail as possible about:

  • Weight loss progress

  • Side effect severity

  • Appetite changes

  • Medication tolerability

  • Reasons for requesting the adjustment

Supporting documentation such as lab results or provider notes may also be uploaded if available.

When should I submit my monthly check-in?

Recommended submission timing

Complete your monthly check-in at least 72 hours before your next scheduled order date.

This allows enough time for:

  • Clinical review

  • Prescription approval

  • Pharmacy processing

  • Shipment preparation

What happens if I submit my check-in late?

If you miss the check-in window:

  • Your refill may process at your current dose

  • Dose adjustment requests may apply to the next refill cycle instead

Late submissions can also delay prescription approval and shipment processing.

What happens if my responses are incomplete?

Missing or incomplete information may delay your order.

Make sure you:

  • Answer all required questions

  • Provide accurate information

  • Upload requested documents where applicable
